Bounded projections retain pending interrupt, approval, and user-input request dependencies and reserve encoded bytes for them. Snapshot windows add matching pending request cards that are not already retained. Tests check card retention, visible items, history bounds, query indexes, and encoded-size limits. |
Cached projection validation packages/client-runtime/src/state/threads.ts, packages/client-runtime/src/state/threads-sync.test.ts |
The client detects pending user-input requests without matching cards and avoids using those projections to initialize thread state. Tests cover disk and retained caches with and without the card. |
Older history-page merging packages/client-runtime/src/state/threadHistoryMerge.ts, packages/client-runtime/src/state/threadHistoryMerge.test.ts |
History-page merging exempts approval and user-input request items from the visibility filter. Tests check retained turn items and derived pending-request state after merging. |

Explanation
The changes stay within issue
#25806. Server projection retention, client history merging, cache recovery, byte-budget handling, and regression tests directly address pending-input reachability. Shared retention of pending approval and interrupt cards supports the same required-card behavior. No unrelated product workflow, default, contract, or schema change is shown.

Explanation
The description clearly explains the problem, implementation, verification evidence, limitations, and linked issue. It does not use the template headings exactly, and explicit maintainer approval or a focused-fix exemption is not clearly stated, but the required information is mostly present.

This provides integration-check evidence for the pinned source. It does not establish that the current PR revision has the same tree. It also does not cover the full test suites or a desktop build on this source. Before merging, confirm that the tested tree matches the intended integration tree and that required CI checks pass. The exact-head packaged macOS app was built, validated, and launched, but cua-driver classified its first window observation as screen_locked and refused all input routes. The initial opening recording initially has a blank history viewport despite loaded message accessibility entries; its final frame shows the later turns. The second recording, after reopening the same baseline app/profile, shows the loaded later turns and the same inaccessible question state. Both records remain pending in SQLite. Initial before-fix opening: linux-before.mp4Before-fix reopening with later turns visible: linux-before-reopened.mp4After, opening the thread recovers “Which optional check should run next?” and “Which scope?”. Both are dismissed through their “Dismiss question without answering” controls. The panel and Input clear fully in the recording. SQLite persists both as resolved, with decision cancel. linux-after.mp4After quitting and relaunching the same exact-head app and reopening the thread, Input and the question panel remain absent. The separate recording includes a provider-status timeout toast in this credential-free synthetic environment; its cause was not investigated. Unproven: normal release startup, hardware-accelerated Linux rendering, an enormous visible question, real provider generation of these pending questions, answer submission/provider continuation, other clients/providers and the reporter's original database. R5, blocker, resolvedI decoded the actual replacement MP4s with physical corex's ffmpeg and inspected full-size frames, including frame zero and the exact last frame. All are 1132x822 at 15 fps.

Exact-head Linux desktop evidence for The first clip starts on the draft view, opens the long thread, recovers Next and Scope, dismisses both through the app, and finishes cleared. The second opens the same thread after a clean app exit and cold relaunch. It finishes fully loaded with no question panel or Input badge. Read-only SQLite confirms both requests resolved with decision cancel. linux80-large4.mp4linux80-large4-cold.mp4Persisted-cache recovery starts on Away. Before recording, the target thread was warmed, then left, and the document reloaded on Away to remove retained memory. The fixture removes only the older Scope card from IndexedDB, keeping its runtime request pending, and reads that state back. Opening the target triggers a fresh HTTP 200 snapshot containing Scope. The recording shows Scope, its UI dismissal, and a reload that stays cleared. Next was initialized resolved in this fixture; this clip claims only Scope's dismissal. linux80-persisted3.mp4Retained-memory recovery uses a separate profile. A one-shot fixture removes Scope's card from the first HTTP response, keeping its request pending. After dismissing the accessible Next question, navigating Away and back without reloading reaches a real live retained candidate. A read-only debugger capture shows that candidate has the pending Scope request and lacks its card. The runtime rejects it and fetches the complete snapshot. The recording shows recovered Scope, dismissal and a cleared reload. The injection intentionally changes one response; an earlier scratch success string saying otherwise was inaccurate and is corrected in the attached harness. retained-cache.mp4The large fixture has 14 completed turns, two pending message-capability questions in turn 1 and a resolved control. It adds about 31.5 KB of filler per user message and 620,029 bytes of hidden title metadata to Scope, rather than an enormous visible prompt. The measured required-card reserve is 621,336 bytes against a 1,048,576-byte policy. Remaining capacity is 412,104 bytes, below the previous 524,288-byte floor, so this exercises the new reserve behavior. Six latest rows are visible, both cards remain dependencies outside that window, and the projection is 1,039,801 bytes. No reporter data or provider session is claimed. Build: Boat job Cache records, test harness and independent video audit Capture limits: native X11 window capture, background accessibility actions, isolated T3 and Chromium profiles. GPU and occlusion/background rendering were disabled to avoid stale captured pixels. Linux keyboard reload failed, so reloads used CDP |

This supersedes the older supplied integration result. The source-review report found no new actionable finding from the inspected main changes. Its note that the job was still running reflects the review’s earlier handoff; the completed output now supplies that result. The source distinction remains important. Android recovery and dismissal passed on physical corex POCO F4, Android14, with server head The real bounded response retained both older question cards outside visible history. Pending dependencies total621,336 bytes, above half the1,048,576-byte policy, while six recent rows remain visible and the projection is1,039,723 bytes. The synthetic fixture uses620,029 bytes of hidden title metadata to exercise the reserve case; the visible prompt is short. The unedited recording starts away from the affected thread, opens it, shows Next and Scope as actionable questions, dismisses both through the UI, then navigates away and reopens to a loaded normal composer without either question. Persisted requests resolve with cancel at01:12:40.686Z and01:12:49.462Z; canonical events6/10 and accepted command receipts agree. Independent decoding and raw-state/source/build audit found no material gap in these claims. android-head80.mp4This records navigation away and reopening, not a cold process restart. Questions and later history are synthetic; no approval card, native provider generation or Android answer submission is claimed. The desktop answer/provider continuation is covered separately. Exact build commands, outcome and raw state evidence follow. iOS runtime verification remains blocked by the simulator control path, so this is not a ready report. …s the history window A Codex question or approval left pending while later turns completed kept its runtime request (so the sidebar showed Input), but the thread snapshot window dropped the turn holding its card. Current-head iOS verification passed. The recording starts before opening the long thread, shows both older pending questions, dismisses NEXT and SCOPE through the UI, navigates to Away, and reopens the target with a loaded, cleared composer. ios-current-capacity.mp4Current-head iOS capacity recovery passed on 2026-10-30, physical shiv Simlock pool, iPhone 18 Pro simulator iOS 27.0. Exact server and client source head 2a49cf0, tree 5d10943f0ea0dc575341fa5ca1c17da775e05a32, clean parent worktree. Compatible native cache fingerprint 953468b560e21a2a7cfb6d86a69eda1e061efb8a, arm64 IOSSIMULATOR, T3CodeDev 2.2.0 com.t3tools.t3code.dev. On shiv job r_od7hveqvbptd4w00, vp run mobile:install ios --prepare exited 0 using local compatible cache. The cache's original build commit is unknown; this is not a freshly compiled native build or release-package claim. Its 323 archive files matched the shiv bundle manifest. Archive SHA256 a5043851d8d4a9822bfc655f829a19c526a7a000db755ee474a376758e24dff0. Parent installed that returned archive through device_open's AgentDevice command from authorized checkout. First install failed worker-not-connected; prescribed --debug retry succeeded. Fresh iOS 27.0 instead of the previously failing iOS 27.1 Duo. Open with --metro-host 100.88.19.114 --metro-port 18086 succeeded; accepted app's Expo developer-menu intro and own pairing confirmation. A stale Close ref was rejected; exact emitted scoped ref retry succeeded. No SSH/simctl/idb UI fallback. No screen_locked, sleep or lock command. Server: vp run dev:server --home-dir /tmp/test-pr/t3code-25806/ios/backend --host 0.0.0.0 --port 18607, PID 721914. Metro: vp run dev:client --port 18086 from apps/mobile, PID 721915. These ran current source without edits. Actual iOS Metro map bd0409f1beebea2ec40936315be436198723a21fd22b608dfe5ac482b031263e matches threads.ts 8ab33308623ca297402d256207587c8bb504c3f3d187447adf05cc8befaabdc2 and threadHistoryMerge.ts 29f99d210a42f113a2a2a0f201a5e6f145f741e0189f36ff20ae054f9968dbca, compared byte-for-byte with workingtree. App visibly loaded paired server fixture. Fixture is synthetic projection data from existing 14-turn DB, copied into isolated backend; event history is not claimed to be real provider execution. Two pending user_input/message-capability questions are in turn 1. Request suffix approval:pending is also user_input, not approval coverage. Seed adds 31,500 history text bytes per user turn and 620,029 hidden Scope title metadata bytes. This exercises dependency reserve above half budget without a huge visible question. No change to production code or client behavior. Before opening: actual HTTP200 bounded response 1,039,966 bytes, projection 1,039,723 bytes, dependency 621,336 bytes > half the 1,048,576-byte policy, 524,288. Six visible recent rows, both old cards in turnItems and outside visibleTurnItems, hasMoreHistory=true. Files bounded-before.json and capacity-summary.json preserve actual response and measurements. Unedited H264 MP4 ios-current-capacity.mp4 is 134.267 seconds, 1206x2622, 30fps. Recording starts on Chats before opening the target. NEXT and SCOPE render with custom-answer controls and Dismiss without answering. UI Dismiss NEXT then SCOPE. It shows cleared composer, back to list without Input, opening Away, then reopening original target to loaded clear composer. No cold process restart claim. Parent personally viewed live screenshots and decoded contact-sheet frames spanning opening/both cards/dismissed/Away/final. ffmpeg full 4,028-frame decode exited 0. Persisted canonical runtime-request.updated events sequence 5 and 9 resolve the two requests with decision=cancel at 12:40:28.806Z and 12:41:00.181Z. Command IDs 2e625b6e-b3e1-49f1-b007-74adb017af29 and e92e98c6-2815-454c-9420-973792250438 have accepted receipts. Final read-only SQL has zero pending; actual fresh bounded GET 200 snapshotSequence=12 also zero pending. No direct SQL resolution was sent, only initial fixture writes while server stopped and UI cancellations after start. Cleanup: record stopped successfully and exported, device_close shutdown=true wiped/returned only own pool lease 62e73da3b5405e9abeddfe0e2ef00aa9/sessiont3-e06306. Stopped own captured server/Metro process groups; ports 18607/18086 both refuse connections. No device app/connection/process remains. Immutable fun outputs and reusable native cache preserved. Limits: simulator/dev client, original native-cache source commit unknown, synthetic older questions/history and hidden title inflation, remote tailnet client/server. No physical iPhone, actual provider question generation/answer on iOS, relay/tunnel, mobile cold process restart, release signing, or mandatory-state universal byte-cap claim. Current desktop separate run proves actual Codex answer continuation. No readiness claim in this artifact. Independent current-head evidence auditCurrent evidence auditNo new R7+ finding. The minimum current-source follow-up in the rebase review is satisfied: iOS capacity recovery/dismissal/reopening, desktop recovered-answer continuation through the original real Codex conversation, and the two focused answer/dismiss integration tests. This is an evidence-review conclusion, not readiness or merge approval. PR #26435. Reviewed 2026-10-30, head Read fleet AGENTS/go/babysit-pr/test-pr/unslop, product ledger and repository Macroscope policy. Reviewed only new evidence and existing finding/reuse dispositions. Tests and stable findings
Historical Linux/cache, rollback, Android and native-question reuse decisions remain unchanged. Keep recordings labeled historical head Parent handoff and limitsPublish iOS evidence, rewrite the PR body around final proof and reserve behavior, then run final live ready check. No readiness claim here; required reviews/checks and current-main mergeability were not reassessed. Keep explicit: synthetic old questions/history and iOS title inflation; unknown original native-cache commit; development desktop/dev-client testing; mock Keychain/isolated desktop home; no current release signing/packaging; no same native blocking-question restart recovery; no iOS real-provider answer or cold mobile restart. Historical Android reopening is navigation. Reporter data, physical iPhone, relay/tunnel, Windows, accelerated Linux rendering and filesystem restoration remain outside these claims. Mandatory state or one oversized row can exceed the byte policy. Review-generated decoded frames and temporary comment copy were removed after inspection. Requested audit report, parent evidence and immutable fun artifacts are preserved. PR #26435, head 2a49cf0, checks and reviews not reassessed; prior green CI reused, parent owns final live ready check. Exact-head focused test outputHead Rebase reuse review: patches 3/5/6 match the final head80 patches; patches 1/2/4 changed only surrounding context. The four changed production files and relevant upstream cache/cursor/answer interactions were reviewed on current head. Historical Linux/cache, actual rollback and Android recordings retain their head80 provenance; current iOS and desktop continuation fill the changed runtime paths. R1/R2/R3 remain resolved, R4 remains disputed with source/tests and real rollback evidence, R5/R6 remain resolved, no R7+ findings. |

Closes #25806
An older pending question could leave Input in the sidebar after its card fell outside the history window, leaving no way to answer or dismiss it. The server now retains pending question and approval cards as dependencies, reserves their bytes before selecting visible history, and keeps them available when paging. Shared client logic also rejects incomplete disk and retained caches so a fresh snapshot can recover missing questions.
The dependency reserve can reduce the visible window while history remains pageable. Mandatory dependencies or a single oversized row can exceed the byte policy.

The same real Codex conversation continues withRECOVERED-fern842; reload keeps the request resolved. Development build, not a packaged release claim.vp i --frozen-lockfilebefore the final two tests passed. An independent current-head evidence audit found no new findings and confirmed the two targeted runtime checks. The audit and test output are included in the iOS evidence comment.

It supports reusing the Linux/cache, rollback and Android evidence with that original provenance; the current iOS and desktop checks cover the remaining runtime paths.Earlier browser reproduction, supplementary before footage:

Limits
Older questions and later history are synthetic fixtures. The capacity case inflates hidden card metadata. Retained-cache footage uses the disclosed one-response injection; persisted-cache footage deliberately removes a cached card. macOS testing uses mock Keychain and an isolated home. iOS runs current Metro sources in a compatible cached native dev client; its original native-build commit is unknown. Mobile reopening means navigation, not cold process restart.
Not verified: the reporter's database, recovery of the same native blocking question across provider restart, iOS provider-answer continuation, a physical iPhone, normal macOS Keychain/home startup, current release packaging/signing, accelerated Linux rendering, Windows, relay/tunnel, or filesystem restoration. The synthetic resolved oversized row also exceeds 1 MiB after dismissal, matching the existing oversized-first-row paging fallback; this PR makes no universal byte-cap claim.
Implemented and reviewed with Opus and GPT-6.1-Sol through Claude Code and Codex on behalf of shivam.
